A TRAUMATIC BRAIN INJURY PROGRAM is provided in Buffalo, Erie county and Niagara county, Western New York, by Phoenix Frontier, Inc., a private, non-profit human services corporation providing assistance to people with developmental disabilities and others with special needs.
WHAT IS A TRAUMATIC BRAIN INJURY PROGRAM?
TBI (traumatic brain injury) is any damage to the brain caused by external physical force such as motor vehicle accidents, falls, sports injuries, assaults, gunshot wounds and may also result from other acute medical and clinical conditions. People with TBI may experience temporary or permanent difficulties with language, thinking, memory, attention, judgment, problem-solving and fatigue.
OUR TBI SERVICES: A Traumatic Brain Injury Service Coordinator will assist the waiver participant in identifying and coordinating rehabilitation supports and services.
INDEPENDENT LIVING SKILLS TRAINING One-on-one support is provided to focus on increasing independence in practical skills, such as cooking, shopping, banking, etc. TBI Rehabilitation Services may include: assessment, training and supervision of, or assistance to, an individual who had brain injury and needs training in any of the following: self-care, task completion, communication, medication management, community transportation skills, mobility, sensory/motor skills, reduction/elimination of maladaptive behaviors, problem-solving skills, money management and maintaining a household.
COMMUNITY INTEGRATION COUNSELING Helps the participant and/or family members fulfill emotional and psychological needs.
INTENSIVE BEHAVIOR PROGRAM Direct assistance is provided to individuals with brain injury to decrease challenging behaviors. Through training, staff develop support strategies to assist participants, family members and caregivers.
STRUCTURED DAY PROGRAM This Traumatic Brain Injury Day Program provides an environment that is focused on recreational and educational activities. These are useful tools for building social skills and helping participants become reintegrated into the community. Individuals have a variety of social, educational and recreational opportunities to choose from, including: dining out, shopping, computer classes, exercise and fitness, swimming, peer counseling, field trips, music appreciation, arts and crafts, picnics, recreation and game room, hiking, and community volunteering.
ELIGIBILITY
Primary diagnosis must be TBI (Traumatic Brain Injured). We offer private services and also services to people who are Medicaid eligible, HCBS/TBI (Home and Community Based Services/Traumatic Brain Injury) Waiver eligible or OMRDD (Office of Mental Retardation and Developmental Disabilities) Waiver eligible, or Long-term Home Health care.
We are authorized by New York State Department of Health to provide services to individuals who have suffered a traumatic brain injury, ages 18-65, living in Erie and Niagara counties.
