MEDICAID SERVICE COORDINATION is a service that is available for individuals who have a diagnosis of developmental disabilities and are enrolled in Medicaid.

Medicaid Service Coordinators (MSC) promote advocacy, fulfillment of goals, and the pursuit of an enhanced quality of life. MSCs provide a multitude of individualized services according to each person's specific needs and circumstances.

OUR STAFF The Medicaid Service Coordinators at Phoenix Frontier, Inc. have earned a reputation for being strong advocates. Our MSC's must meet annual, on-going training requirements. We have a consumer ratio of 1:25.

The MSC's at Phoenix Frontier, Inc. strive to create a partnership with the individual and his/her other supports, such as family, friends, teachers, advocates, medical and other service providers. Our focus is on assisting each person live the life they choose. Our MSC professionals are committed to providing services that encourage independence, inclusion, productivity and individuality.

ELIGIBILITY Individuals must have been diagnosed with a developmental disability (mental retardation, cerebral palsy, autism, epilepsy, neurological impairments) that occurred before the age of 22 and will continue for life, and the individual must be Medicaid approved.

SERVICES AVAILABLE

For individuals who are eligible for service coordination, we offer the following services:

Develop an individualized service plan (ISP)
Assist in finding health services
Assessing entitlements and benefits
Obtaining Respite/Recreation Services
Obtaining affordable and quality housing
Researching the need and applying for environmental modifications
Crisis intervention
Promote self-advocacy
Provide linkage to employment opportunities
